Raul may miss Spain's World Cup quarter-final against Italy or South Korea because of a groin strain.
He was substituted during the second-round clash with the Republic of Ireland, which Spain won 3-2 in a penalty shoot-out.
"He (Raul) will be under observation for 48 hours with plenty of ice and rest," said team doctor Genaro Borras.
"It's not clear whether he'll make it for the quarters."
Raul was substituted in the 80th minute after a collision with Gary Breen.
Midfielder David Abelda also looked in doubt.
Borras said he was taken to hospital and diagnosed as having a 'twisted testicle', which he apparently suffered minutes after taking the pitch as a 71st-minute substitute.
Borras did not say how the injury occurred.
