Tallaght chief was aware of x-ray problems last summer

The head of Tallaght hospital Kevin Conlon has confirmed he was aware of GP concerns about radiology at the hospital last summer.

But it only emerged last night that 58,000 x-rays taken over a four-year period were never reviewed by a consultant radiologist.

One patient has died and another is now being treated for cancer.

The HSE has launched an investigation into the blunder, which will be chaired by an independent figure.
