Cancer campaigner Rebecca O'Malley has praised the HSE for what she says is an unprecedented demonstration of greater openness, transparency and acceptance of accountability for patient safety.
The cancer survivor was reacting to the publication of an implementation plan by the HSE following recommendations made by the Health Information and Quality Authority.
Ms O'Malley was mistakenly given the all-clear for breast cancer in 2005 before being correctly diagnosed with the disease 14 months later.
She says the HSE's action is evidence that essential changes are beginning to occur and that patients will eventually benefit from safer care and treatment in respect of symptomatic breast disease.
