A private nursing home in the Midlands was found to be in breach of the Health Act on a number of grounds when inspected by the Health Information and Quality Authority earlier this year.
The HIQA inspection at the Gallen Priory nursing home in Ferbane, Co Offaly, was made on foot of information about poor nutrition in the centre.
During a visit in mid-August, inspectors found there was nobody in charge of the nursing home and care assistants with no formal qualifications provided the majority of direct care.
HIQA found that many residents spent their day in a large day-room with a lack of supervision for extended periods and that residents had no forum to influence aspects of life.
The building, a former convent, was too old and unsafe for residents to walk around.
Some residents were subsequently restrained as they were deemed to be at risk of falling.
HIQA also raised concerns about infection control and cleanliness at the home.
A follow-up visit in September found that the majority of the inspector's immediate concerns - such as cleanliness and management - had been addressed.
