An independent report into the death of a 75-year-old man at Monaghan General Hospital last October is due to be published later today.
Patrick Joe Walsh, from Killanny in Carrickmacross, bled to death at the hospital after doctors were unable to transfer him elsewhere for emergency surgery.
Mr Walsh was suffering from a bleeding ulcer, but staff were unable to get a bed for him at Cavan General Hospital and at Our Lady of Lourdes Hospital in Drogheda.
It later emerged that intensive care beds were available in both hospitals.
Today's report into the matter was commissioned by the Health Service Executive and compiled by two independent experts.
Their preliminary report concluded that there was "systematic evidence of serious process failure at almost all levels of activity, apart from nursing care".
