If Savita Halapannavar was given a termination, she may still be alive today, according to the head of the HSE inquiry into the circumstances surrounding her death last year.

It comes after the Health Service Executive and University Hospital Galway apologised unreservedly to Savita Halappanavar's husband Praveen and her family.

The report highlighted three main factors in the 31-year-old’s death from blood poisoning following a miscarriage.

Professor Sabaratnam Arulkumuran has said the medical staff were concerned more with the foetal heartbeat than the life of the mother.

He said legal clarity is needed to stop another incident from happening again.

The HSE and UHG have said they will fully implement all the recommendations in the report.

Dr Patrick Nash, the Clinical Director at University Hospital Galway, said: "On behalf of University Hospital Galway I would like to sincerely apologise to Praveen Halappanavar and family for the events related to his wife's care that contributed to her tragic death.

"University Hospital Galway is committed to operating to the highest standards, we can reassure all concerned that we have already implemented changes to avoid the repeat of such an event."
