A Dublin man has revealed how he received an x-ray appointment for his wife, who died 14 years ago.

A letter arrived from Tallaght hospital this week, advising that Linda Dale, from Drimnagh, had an appointment there on August 6.

Her husband John has told the Irish Independent he wants to know how the blunder occurred, but has got no explanation yet.

Tallaght Hospital said its Patient Advocacy Department is available to speak to him and his family.
