Almost 86,000 near misses reported last year


Almost 86,000 'adverse events' at the country's hospitals and community health centres were reported to the State Claims agency last year.

They are classed as near misses - where no harm is done to the patient - and range from delays in access to damage to a patient's health.

Slips, trips and falls are by far the most common adverse events and account for 32% of all events reported in 2011.
